Terms extend to 25 to 30 years, reflecting the long-term stable nature of NHS contracts. Rates typically sit at prime commercial levels for well-structured propositions.

Dental practice finance covers practice acquisitions, associate buy-ins, and premises purchases. NHS dental contract dependency affects lender appetite: mixed NHS/private practices are typically preferred over purely NHS or purely private practices, reflecting balanced risk profiles. Goodwill valuations for dental practices typically run at 100% to 150% of net profit for established practices, with sector-specialist valuers (including Christie and Co, Colliers Dental) commonly instructed.

Care home and other sector finance

Care home finance is one of the more complex healthcare sub-sectors. CQC rating materially affects lender appetite: ‘Outstanding’ and ‘Good’ rated homes attract mainstream commercial pricing, while ‘Requires Improvement’ or ‘Inadequate’ rated homes are typically declined by mainstream lenders. Sale of larger care home operations regularly involves structured finance combining term debt, mezzanine, and vendor loan notes. Typical care home mortgage LTVs range 60% to 70%, reflecting the operational risk in the sector.

Veterinary practices, pharmacies, and opticians all have sector-specific lender treatments. Pharmacy finance in particular benefits from specialist lenders (Praetura Pharmacy, Wesleyan Pharmacy) that understand the regulated dispensing income model. Veterinary practice lending has seen significant activity since the corporate consolidation trend of 2018 onwards, with specialist providers now handling most veterinary practice transactions. In each sub-sector, the sector-specialist route typically produces materially better terms than general commercial lending.
